New Hire – Onboarding Form

The form below allows WisPIC IT staff to enter new employees' information into our management systems as we as to make assessments about their potential technological needs. All supervisors must fill out the form on behalf of new personnel, which includes administrative staff, providers, faculty, interns, research staff, etc. With the information provided we can more efficiently allocate necessary resources and promptly fulfill requests for access to shared resources.

We strive to make the technical onboarding phase as streamlined as possible for the newest members of the Department of Psychiatry.
