How this form helps your IT Team
When we receive an offboarding form, that is our cue to verify that the outgoing person’s access to any Restricted/Research/Shared drives is set to expire or is to be removed, likewise with any service accounts. We also check our records as far as any university-owned equipment that was issued.
Generally, the form below is for their direct supervisors to fill out. However, lab managers or principal investigators may also take the initiative and fill it out.

Additional steps and information
Ensure that the outgoing employee has returned their laptops, chargers, and any supplied peripherals (mice, keyboard, webcam). If they worked on a shared desktop on premises and/or remotely, there is no further action need. Further, please direct the outgoing employee to drop off equipment at either WisPIC room 1202 or 1220. Alternatively, you may collect and return the equipment yourself.
For any individually-assigned desktops, verify that it is located in their assigned work location.
