An Important Update Regarding Request Forms
The form you are looking for is no longer in use. Going forward, please use the new SMPH Support Portal, where you can submit the following types of tickets:
- General IT Help
- Onboarding for new employees (staff, researchers, interns, residents, faculty, student help, etc.)
- Offboarding forms to remove access
- Service & resource accounts
You can still send an email to support@psychiatry.wisc.edu and use the above linked portal to monitor the progress of your request(s).
